To apply online for a Swavalamban card, you need to follow the steps below:
- First, visit the official website of the Unique Disability ID – http://www.swavlambancard.gov.in/.
- After that, a new page will open on the screen.
- On this page, you should click on the link “Apply for Disability Certificate and UDID Card“.
- By clicking here, a form will be opened in front of you.
- Now you read the instructions and fill out the online application form.
- Upload all required documents like Aadhar Card, Caste Certificate, etc.
- Then next, send the application form to the CMO / Medical Commission office.
- This way, you can apply.
How to apply offline for a unique disability ID card

All states/UTs have started creating UDIDs.
- Persons with Disabilities (PwDs) have already started applying online for Certificate of Disability / UDID Cards from all States / UTs.
- A registration/registration number will be generated for all successfully submitted applications.
- An SMS confirmation will be sent to the applicant along with the registration/registration number.
- Applicants who do not have a certificate of disability will receive an SMS containing the date and time to visit the nearest CMO office for assessment.
- The disability certificate / UDID card will be issued online after evaluation and verification.
- An SMS confirmation will be sent to the applicant with the UDID assigned.
